We look forward to meeting you. Please arrive 15 minutes early for your appointment to make sure you have completed the appropriate forms. When you enter Onelife Fitness gym, check in at the reception desk and they will point you in the direction of the clinic, which is to the right after entering the front doors.

What should I wear?

Basic athletic or workout attire is appropriate and helpful, especially for the initial evaluation.

How long is each session?

Both the initial evaluation and follow up treatments are 45 minutes long with the same therapist each time. For your first session, the physical therapist will evaluate and discuss your current condition or injury, including specific hands-on tests to find out what’s contributing to your issues.

Do I need a prescription from a doctor?

In the state of Virginia, physical therapy services can be provided without a physician referral for up to 30 days. After that time period, if you need further care, a physician script for physical therapy must be provided. We are happy to communicate with your doctor and/or provide documentation notes if needed. However, some insurance companies dictate that you must have a physician’s script to pay for physical therapy services.

Who chooses where I go for Physical therapy?

You are the most important member of your healthcare team and you have the right to choose who you want to provide your care to meet your goals

How many visits will it be?

After your evaluation, your physical therapist will give you an estimate of how long they anticipate it will take to get you better. Please speak with our office manager if you have questions about any visit limitation that your insurance provider dictates.

What portion will I be financially responsible for?

You will be responsible for your insurance deductible and/or co-pay or co-insurance.

How do I find out about my deductible and co-pay?

You will receive your insurance benefits at the time of your appointment.

Please bring the following documents:

  • Download new patient forms, print, and complete
  • Bring current health insurance card and valid ID
  • Bring your prescription from your referring physician
  • Bring x-ray or MRI reports, and/or physician reports related to your injury
